Mitkof Island is defined by a landscape of steep ridges and tidal waters within the Tongass National Forest. The geography is anchored by substantial elevations such as E Sumner Mountain and S Sumner Mountain, with Sumner Pass providing a natural corridor through the heights. To the east, the island meets the Sumner Strait, while the interior is carved by the Blind River and the complex tidal basin of Blind Slough.
