Ismay sits near the northern boundary of this landscape, where the town site meets the high plains terrain of Eastern Montana. The area is defined by a dense network of seasonal drainages, including the bifurcating paths of N Fork Cottonwood Cr and S-Fork Cottonwood Cr which dominate the central sections. Significant for local historians, the Ismay Cem is located just north of the township near the banks of O'Fallon Creek.
