Ismay sits at the northern edge of this high plains landscape, situated near the intersection of Whitney Cr Rd and Ismay Rd. The town's proximity to the winding O'Fallon Creek drainage defines the local geography, with the Ismay Cem positioned just west of the settlement center. The terrain is a complex network of seasonal watercourses and coulees, including Station Creek and Sandstone Creek, which feed into the larger creek system.

Tracing south across the border between Custer Co and Fallon Co, the land rises toward the prominent Twin Buttes. This southern portion of the quadrangle is dominated by the many branches of the Cottonwood system, including North Fork Cottonwood Creek, South Fork Cottonwood Creek, and the primary Cottonwood Creek. The presence of Scroggin Creek at the far southern boundary illustrates the typical drainage patterns of the eastern Montana rangelands, where water remains the primary factor in land use and settlement history.
