The river's influence is central to the landscape, from Babbs Island near the Jennings Randolph Memorial Bridge to the small settlements of Walkers and Newell further downstream. Higher ground is defined by family-named landmarks like Stewart Hill and Locust Hill, while the Kent State University East Liverpool Campus sits at the heart of the urban cluster.

South of the river, the terrain transitions into a network of winding runs and wooded hollows, such as California Hollow and Laurel Hollow. Significant local history is preserved at the Locust Hill Cemetery and the Tomlinson Run United Presbyterian Cemetery. The southern reaches are more rural, punctuated by the town of New Manchester and the recreational waters of Tomlinson Run Lake, which is contained by the Tomlinson Run Dam.
