Smithville and the nearby settlement of Nani-chito are tucked into the southeastern corner of the Ouachita National Forest, where the terrain transitions from the heights of the Boktuklo Mountains toward the winding waters of the Mountain Fork. This area of McCurtain County is defined by its deep historical roots, evidenced by a concentration of cemeteries including the Old Smithville Cem, Nani-Chito Cem, and Big Lick Cem. The landscape is a network of ridges and watercourses, with Big Eagle Creek and Boktuklo Creek draining the timbered slopes. Notable geographic features such as The Narrows indicate where the topography constricts the river's path. Traces of older transit ways survive in the form of the Wild Horse Boktuklo Trl and E Boktuklo Trl, which navigate the high ground west of the primary valley settlements.
