Otway sits at the northern confluence of South Fork Scioto Brush Creek and Bloody Run, serving as a central hub for the surrounding ridge and hollow topography. This part of Scioto County is defined by its deep, narrow valleys where family names are etched into the landscape, visible in burial grounds like Parker Cem, Moon Cem, and Euton Cem. To the east, the settlement of Henley follows the curve of Scioto Brush Creek, while Lombardsville anchors the southeastern corner near Saw Pit Run. The southern portion of the area transitions into recreational and conservation land, featuring the Backpack Main Trail N Loop and N Country National Scenic Trail which wind through Hobey Hollow and past Bear Cr Lake. The network of township highways, such as TWP HWY 112 and TWP HWY 111, highlights the enduring rural layout of these communities.
